Star Ocean Til the End of time: Sidetracked, I came back to it weeks later then I get sidetracked again, repeat a few more times until I forgot most of the story.
Xenosaga Episode II: Sidetracked like I did with Star Ocean 3.
Haunting Grounds: I got bored of the game after the Chapel.
Silent Hill 4 The Room: I was near the end with low health and no health drinks, I think the game's antagonist who was chasing me killed me before I can get to where I was suppose to go.
Tales of Legendia: Sidetracked by Ragnarok Online.
Tales of Eternia: Volt Glitch happened, I downloaded a file that completed the volt dungeon but it just didn't feel right for me to leave my save file.
Tales of Symphonia: Dawn of the new World: I enjoyed it, I got pestered by someone else who want to use the Wii to either play games or watch something on Netflix.
Harvest Moon Animal Parade: Same as above.
Harvest Moon A Wonderful Life: Loan it to my niece now it's either somewhere in their house or it's gone.
Grandia Xtreme: It was too much of a grindfest even for me.
Grand Theft Auto 3: I took a break and loan it to my cousin, never got it back. I'll have to get that triple pack to replace it along with Vice City.
Grand Theft Auto San Andrea: Can't remember why.
Atelier Rorona: Either I put it down to play another game or my brother borrowed the PS3 to play CoD with his buddies that only owns PS3.
Lunar Dragon Song: Yup I added this game to this list. I got fed up with the battle system.
Bioshock: I played this to kill time until the release of a few new games, lost interest to finish the game after finishing the new games.
Persona 3 The Answer: I think I was halfway into the story when I just got sick of the game at that point, I came back to it later and forgot what happened so far.
The rest are mostly RPGs games I rented on the older consoles like PS, PS2, SNES and Sega Genesis. Then there are the translated roms.
Good one, I'm guilty of this as well. I loved the first one and popped II in immediately after the end of the first and my reaction was exactly this:
Could not get more than an hour into it, and never went back. Poor Episode III, I heard you were so good too. Maybe one day... maybe one day...
Xenosaga Episode II was not that bad...Well, I guess that I can understand if you played it RIGHT after Episode I.
Also, Zelda II is not a bad game. <_< It has some flaws, sure, but it is far from a bad game.
Anyways, not counting games that I still have the ability to beat at this time, I guess I would say: Quest 64, Jet Force Gemeni, Tomb Raider, and... any of the Army Men games on the N64. Games from that time, back when I used to rent games (was able to rent games).

Final Fantasy XII - The story was so bad that half way through I forgot what I was even doing in the game and couldn't figure out how to progress as a result.
Persona 3 FES - Accidentally missed getting a very useful item that could only be acquired on one day, lost all interest after realizing that.
Bioshock - I have the PC version, it crashes a lot so I eventually gave up.
Xenogears - I must have started playing this game at least 5-6 times over the years, and always got side tracked.
Shin Megami Tensei Nocturne - I did a poor job of distributing my points at each level up, and had to throw in the towel late into the game.
